The first of the modern ships to be completed with dual purpose guns was the Deutschland. Fitted with 10x5" and 10x37mm, single 20mm were added later as they became available. Control of the 5" was through the three 3m AA directors with a low angle director on the conning tower.

One of the more interesting kit bashes I have done was the conversion of a Scharnhorst with 9x11" to 9x13.8". With the original I left the 5.9" and 4.1" of the original armament.

With a lot of these DP conversions, I will put the 5" where previous weapons have been as this means there will be ammunition handling facilities in the same place.

A dual purpose Scharnhorst with 13.8" would be a formidable opponent for the British Battleships and battlecruisers of the time. The saving made by having the DP armament over the mixed secondary/tertiary guns, put into extra armour would also help. The standard Scharnhorst with 11" is 785 feet, while the 13.8" version is 802 feet. The 11" weighs in at 31,800 tons standard, the 13.8" at 36,000 tons standard (more breadth, more armour).

This is a very interesting topic, I'm looking forward to more. I had some ideas towards this topic for a while, but nothing ever came of it.

Fun fact: The "almost-final"-design I/M 26 for the Deutschland-class was planned to have four 120mm twin DP mounts.

I am not sure if the 12.8 cm FlaK 40 (barrel caliber 61) would be a Gun which could be used as dual purpose gun for ships. That gun was used as heavy AA, also in a twin arrangement on Flak (AA) Towers.
Though it's a quite heavy weight gun. The single gun alone has a weight of 17 tons.
I guess for a Destroyer, 4 such twin towers would be maybe too heavy. I guess build in a turret it would weight around 50 ts.
